Adults: 1 to 2 gm 2 to 6 times a day. Childern and Neonates: 50-200 ma/day in 3 to 4 divided doses. 

Prophylaxis: 1 gm 30-90 minutes before surgery followed by 1 gm 6 and 12 hours after first dose
